FormConnect is an easy to use app for designing and filling out forms for use on your iPhone and iPad. Its simple design allows you to customize the forms the way you want them to look.
The app comes with several pre-built templates to choose from. Pick whichever one you like or build your own. Customize your form to accept text, dates, check boxes, pick lists, radio buttons, photographs and signatures fields.

Attachment PROBLEMS
Whenever I attach pictures and click on done , it automatically goes to job list. It’s really very time consuming to search for job and go to the last page again....
It’s hard to do same things again and again as I use this app 5-8 times a day to make JSA’s

We have been using forms connect since 2013 and have found the program very adaptable to our changing business. Templates are easy to use and change, which makes this great for our day to day reporting.
It's great
This app is just what I was looking for. I have been looking at a lot of the competition, but there isn't anything I could find that could export a PDF. Numbers failed me, as did Bento but this is a fully self contained app that does what it says. You create the fields you want and where you want them. PDF outputs work via email or to any number of PDF supporting apps on iPad. You can also export to various other formats. I really like the drop down lists where you create the options, and times and dates are displayed "now" or they can be changed to what you want. I only expect this to get better and highly recommend it.
