PUMP is an app designed to connect patients, nurses, doctors and relatives
together to help manage heart failure.
For patients with ‘heart failure’, where the heart cannot match the body’s
needs, there are many things to keep track of. Fluid intake, weight,
medications, blood pressure, medical appointments, test results – the list
goes on. We developed pump together with patients to manage all of these –
and also keep track of things we couldn’t do easily before, like activity
levels, mood, and symptoms that develop.
PUMP will remind you to take your medications and keep an accurate list,
and will also let you know before your appointments. We’ve also programmed
in a few algorithms to keep your blood pressure and weight under control,
all under a simple clear interface.
PUMP allows you to centralize all of your care for your heart failure – and
if you choose, you can share it with your heart failure team to help us
help you. If you call to let us know about a problem, we can look up your
record and see all of the data you have been entering, to make a better
