Description
Personen met een langdurige of intensieve zorgnood krijgen vaak hulp van meerdere zorgaanbieders tegelijk. Denk maar aan professionele zorgaanbieders, welzijnswerkers, maar ook familie, kennissen of andere mantelzorgers.
Dankzij Alivia kunnen al deze zorgaanbieders via de app makkelijk een zorgplan opstellen waarmee ze doelstellingen bepalen, taken verdelen en informatie uitwisselen. Hierbij staat de zorgbehoevende altijd centraal. Wat vindt de zorgbehoevende zelf belangrijk in de dagelijkse situatie, wat is hier precies voor nodig en wie neemt daarin welke taken op?
Dankzij Alivia is het voor iedereen duidelijk wat er verwacht wordt. De persoon die zorg nodig heeft, is mee aan boord en behoudt zo altijd de belangrijkste stem bij elke beslissing.
Zo zorgt Alivia voor een echt én hecht zorgteam.
