Description
Deze applicatie laat toe aan ziekenhuizen en zorgverleners om hun patiënten een digitaal zorgtraject te geven die hun optimaal informeert gedurende het hele traject. Er is de mogelijkheid om extra medische informatie te raadplegen op maat van de patiënt/behandeling alsook artsen/zorgverleners op de hoogte te houden van hun gezondheidsstatus door middel van vragenlijsten. Deze applicatie is aldus gebouwd om de patiënt beter te informeren en voor te bereiden op een ingreep / start therapie en de informatie/communicatie arts/patiënt te optimaliseren. Deze beide punten staan centraal in patiëntgerichte en kwalitatieve zorg.
