Stroke prevention for your patients with atrial fibrillation

The Canadian Cardiovascular Pharmacist Network (CCPN) SPAF Tool App is designed to assist Canadian healthcare professionals optimize stroke prevention treatment in their patients with nonvalvular atrial fibrillation. It was modeled after the CCPN SPAF pocket card. It incorporates stroke risk stratification using CHADS and CHADSVASc criteria, bleeding risk using HAS-BLED criteria and provides recommendations for stroke prevention therapy based on the Canadian Cardiovascular Society (CCS) AFIB Guidelines. This app also includes practical drug information, dose recommendations, creatinine clearance calculations, and management tools which allow healthcare professionals to compare therapeutic options and tailor therapy for individual patients. It is intended as a general reference to supplement the existing knowledge of healthcare professionals.

This app was supported in kind by the Centre for Innovation in Complex Care (CICC) and through an unrestricted grant from the Pfizer-Bristol Myers Squibb Alliance and Servier Canada. The original CCPN SPAF pocket card was supported through an unrestricted grant from Boehringer Ingelheim Canada
