Best allergic reaction & treatment tracker app for KIDS

Been looking everywhere for an app that I can use to track when my 9 month old son has an allergic reaction, include what it might have been to cause it and if there’s any treatment administered, sorta like I could keep this log or diary for his paediatrician and I have had no success until now! This app is awesome! Highly recommend!
