With the present application, the Hellenic Cardiological Society offers any patient with heart failure the opportunity to become aware and trained in the self-monitoring and self-management of its disease, always according to the instructions of his treating physician. Patients are given the opportunity to take an active role in the management of heart failure. The patient focuses on monitoring key vital signs, symptoms, and activity.
The first step is to enter the patient's profile information. Then, the patient has two options: the daily recording and the monthly one. On a daily basis he/she will add elements of his blood pressure, body weight, heart rate and will choose the type of symptoms that may or may not occur.
There will be a question to remind him/hervof taking his medication as well as to record the reason for taking it incorrectly, in order to educate him on the importance of compliance in treatment. Any abnormal report, either as a symptom or as a measurement, will prompt you to contact your doctor. Also training material for heart failure will be available through the application. All data is stored on the patient's device alone, who is the only one who can manage it. Only if he wishes he can share them as a report in PDF format with his doctor.
