Description
CGI CC360HC is a solution that takes care to the palm of your hand, with exact information you need to take decisions. CGI CC360HC is a mobile health platform empowering citizens with long term conditions to monitor their personal health data. The solution supports cross provider care keeping the citizen into center and allows customizations for an individual citizen. CC360 app integrates with the HealthKit app. They can submit measurements using HealthKit enabled devices. By using the Health app, data is fetched to access the current measurement data and also use the system to communicate with the patient directly through email messaging or video call within this app. Data registered by citizens is available to professionals and caretakers in realtime. The app supports involvement of the family and community in the care of citizens.
