I have installed this app , but I’m not able to sign in . The app ask email id and phone no but not able to sign in. So please resolve asap

Dear Vikas, the ContinuousCare Application can only be used once your Healthcare Provider adds you to their Virtual Practice or you have self-registered using their Web Patient Portal, as it is not a standalone app for patients. You would then be able to log in using your patient user account, to connect with your preferred Healthcare Providers.

As explained in the app description, the ContinuousCare application can only be used if your healthcare provider has added you as a patient in their Virtual Practice system. You will then be able to log in using your patient user account.
