Adolescent HIV Transition Readiness Assessment

eHARTS is a transition readiness calculator for adolescents living with HIV who are transitioning from pediatric to adult based care. The calculator is based on a transition readiness score and the HIV Adolescent Readiness for Transition Scale (HARTS). Both of these assessments have been validated based on viral suppression rates 12 months after transitioning to adult care for adolescents living with HIV in South Africa. The eHARTS is intended for use by clinicians to determine an adolescent's individual readiness to transition based on three levels: high, intermediate, and low. Both clinicians and adolescents complete a brief questionnaire. eHARTS then displays the results of their transition readiness in addition to areas where they may need improvement. An individual's scores can be saved and tracked over time.
