Description
Luchtbrug is een online tool waarmee we een deel van de poliklinische controles vervangen door online zorg. Afhankelijk van hoe het met je gaat, hoef je minder vaak naar de polikliniek te komen. Je ontvangt via Luchtbrug regelmatig een korte online vragenlijst waarmee we volgen hoe het met jou gaat, dit wordt ook wel online monitoring genoemd.
Via Luchtbrug kun je ook op een makkelijke manier contact zoeken met je zorgteam en jouw individuele zorgplan inzien. Daar vind je alle informatie over medicatie, doelen en een actieplan in geval van klachten.
