Patient Journey app begeleidt patiënten en hun naasten voor, tijdens en na de behandeling met de juiste informatie op het juiste moment. De app wordt gebruikt door meer dan 100 zorginstellingen over de hele wereld, voor veel verschillende behandelingen.
De app stuurt push notificaties over belangrijke handelingen en wanneer nieuwe informatie beschikbaar is. U kunt de app ook gebruiken om uw persoonlijke voortgang bij te houden en gezondheidsuitkomsten als pijn, functie, activiteit, tevredenheid, gewicht, hartslag en kwaliteit van leven te monitoren.
Wetenschappelijke studies die met de app zijn uitgevoerd tonen een significant effect op bijvoorbeeld patiënt tevredenheid en kwaliteit van leven.
Na opstarten van de app kunt u uw zorginstelling en behandeling selecteren. Indien uw zorginstelling u heeft voorzien van een persoonlijke code, volg dan de ontvangen instructies over het gebruik van deze code.
Belangrijk: Deze app is een aanvulling op uw behandeling. Neem voor medisch advies altijd contact op met uw zorgverlener.
Het scherm kan nu mee draaien bij het invullen van vragenlijsten.

- Intended Use
Patient Journey App is a cloud service intended to support healthcare professionals in offering remote care and monitoring services.
Healthcare professionals can develop their own patient journeys within the platform. These journeys may include data collection (e.g., PROMS, PREMS, vital parameters, photos), signals based on decision trees/algorithms, educational content (e.g., disease and treatment information, self-management competences, pre- and rehabilitation exercises), push notifications, reminders, and communication functionality (e.g., chat of video conference). Healthcare professionals may configure such a journey in a timely manner, meaning patients receive all of the above at a time that is most appropriate given the phase, state or duration of their treatment.
Patients are linked to one or more journeys by their healthcare professional and can download the app. From the app, they receive the requests from data, educational content, push notifications and reminders on their personal timeline. In addition to the timeline, the app offers an information library, contact and communication functionalities and a personal profile in which they can configure the app.
Monitored parameters
The specific parameters monitored in PJA are configured by the healthcare provider for each patient journey. PJA provides generic components to collect patient-reported outcomes/experience (PROMs/PREMs), quantitative values (e.g., weight), and photos, and to apply configurable thresholds and signals. - Veiligheidsinformatie

Keep your login details and any one-time passwords (2FA/OTP) private; do not share them. Report suspected misuse at your healthcare professional.
