Switch Tables offers a guide for physicians and pharmacists in switching antidepressants and antipsychotics and in combining mood stabilizers and the conversion of benzodiazepines. It is a helpful tool to calculate the appropriate dosage when switching and/or combining psychopharmaceuticals and to learn the possible consequences of the switch and/or conversion.
Switch Tables aims to improve psychopharmacotherapy and medication safety for all patients. Switch Tables will be updated on a regular basis.
Walter Broekema (1953) is the editor of Switch Tables. He is a hospital pharmacist He graduated as a pharmacist in 1979 and gained his specialisation of hospital pharmacist in 1991. He is former president of the Special Interest Group Psychiatry of the Dutch Association of Hospital Pharmacists (NVZA/KNMP).

A must for physicians, especially psychiatrist
Switching tables is an easy to use tool that helps physicians to switch from one drug to another in the treatment of a psychiatric patient. Normally, it takes time to explain to patients how to switch, but this app gives directly a very clear and also visual picture how to switch from e.g. one antidepressant to another. This will save time and it looks also very professional towards the patient.
The app offers switching tables for antidepressants and antipsychotics, and a table how to combine mood stabilizers. Drugs are organized in a list box, so typing the name of the drug is not necessary.
If a drug is selected the app also offers direct links to ATC-code, PubMed, PubChem and other databases for background information about the drug.
An extra function is the Benzo Conversion in which every benzodiazepine was granted an equivalence factor in comparison with diazepam. As a psychiatrist, I use the switching tables very frequently and patients are content with a printout of the graph that shows day by day, what to do.
The app is limited to antidepressants and antipsychotics.
