My Plan to Flourish is a person-centered planning app for a connected (and mobile) world.
For the person-served, it is a talking vision board they can carry in their pockets that helps support staff learn important things about them like their interests, likes/dislikes, and how to best work with them to optimize plan results. This creates more meaningful and effective interactions and increases client level of satisfaction with themselves – and their relationships!
For healthcare providers, Flourish provides an effective platform to establish succinct, measurable, and achievable goal-setting for the people they serve. It’s mobility and customizable interface provides a more engaging way keep clients involved in working toward and achieving goals between healthcare visits or check-ins.

- Provides supporting documentation necessary for annual Medicaid waiver.
- Provides a great tool for onboarding new DSPs and staff; helps them acclimate to the people they will serve more effectively and efficiently.
What’s New
Version 2.9.3
- Added a setting that will change the standard Today page into a standalone To-do list. This feature will also move the Goal Steps into the Near-term Goals and Goals from Others pages.
- UI tweaks and bug fixes.
- Updated the app framework to be compatible with the latest versions of iOS and iPadOS.
