Description
The SNAP (Survivorship Needs Assessment Planning) App provides tailored resources, education and support for head and neck cancer survivors and their caregivers as they transition from active care to survivorship care. The app provides symptom management tips, supports emotional and physical well-being, features peer and provider videos, goal making, nutritional tracking, recipes, and more, with the goal of easing the transition to a new normal.
