Fixed, appreciate it so much!

My lists have appeared, but the numbers next to the list names always is 0, and when I click into the list, there’s nothing inside. And some times the app cloudn't even open, it's showing the sync icon and stay there for super long. Only occasionally works properly, but most of the time doesn't.

Thanks so much for reporting this, and apologies that you had this issue with the watch app. We've now found and fixed the bug and released a new version of Actions, so please update to the latest version and the watch app should work fully for you again.
