Download and install a server-version of the simple app in your PC or Server.
Make your form with logic, navigate rules and script.
Add control tags and send done form to your iOS device.
After sync you will see your form on your device and can start to fill in.
Make your surveys, polls or votes and send results from protection link (http or https) to your server or PC.
Then you can export result to txt, csv, Excel, SPSS, MS Power BI and other analytical apps.
