Description
■二次元コードの取扱いについて
接種証明書の二次元コードには、氏名、生年月日、接種記録等の個人情報が含まれます。
取扱いには十分ご注意ください。
■できること
日本国内用と海外用の新型コロナワクチン接種証明書を取得できます。
取得した接種証明書は、アプリを起動すればいつでも表示できます。
他のスマートフォン等で表示された接種証明書の二次元コードを読み取って、内容を確認することができます。
■新型コロナワクチン接種証明書とは
新型コロナワクチン接種証明書は、日本の予防接種法に基づいて各市町村で実施された新型コロナワクチン接種の事実を公的に証明するものです。
氏名、生年月日、接種記録（ワクチンの種類、接種年月日、ロット番号など）が記載されます。
それらに加えて、海外用では、パスポートの国籍や旅券番号が記載されます。
接種証明書には、二次元コードが記載されます。
二次元コードは、氏名、生年月日、接種記録等の情報を含みます。
これを読み取ることで簡単に情報を確認することができます。
■ ご利用に必要なもの
①マイナンバーカード
②マイナンバーカードの券面入力補助用暗証番号（4桁）
③パスポート（海外用を発行する方）

・NFC Type B 対応端末
・iOS 13.7 以上

The passport number issues were fixed for me.
As of the most recent update, I was able to get a certificate with the passport number as all numbers (rather than misreading as letters) — thank you!
I’ve given 4 stars because it’s only available in Japanese, but it works as expected now, and I’m glad it’s on the US App Store regardless.
It works
Gotta say, downloaded this app and had my digital vaccine passport in like 5 minutes. Note I was using this app in Japanese; if you don’t speak Japanese I am not sure it is as easy to use.
