CheckMyVitals is a contemporary method of chronic disease management for Diabetes and Hypertension. It allows patients, providers, dietitians, and other clinicians to connect securely as well as create treatment plans for better organization of chronic diseases. It aims to reduce office visits to the doctor's office and create a new method of operation involving patients actively as part of their own treatment plans. In addition, it also integrated seamlessly with your Health App data and the Health Kit App. These kinds of integrations allow more accurate planning of treatment plans for the patients.

Could be better
Time stamp is wrong on the diet postings. Descriptions of meal do not show up in the chat postings of diet items. Annoying that you cannot use the app at all offline.
Won’t work since this new update
Since it updated within the last few days, I open the app and it just shows the loading screen. It never loads or allows me to sign in or use anything.
Update
Since updating , am not able to input any of my vitals. App says update available.
