Description
The iCHOOSE mobile application and corresponding website is expected to serve as a decision support tool for nephrologists, primary care physicians, social workers, nurses, and/or patient educators to use with ESRD patients to help improve informed decision making for kidney failure treatment options. Risk prediction models for mortality on dialysis of kidney transplantation are presented.

Great app for ESRD patient
Found this app when I started dialysis 5 years ago. While my results from this app are not that good it has motivated me to be compliant with attending my dialysis sessions and I religiously check my lab results to modify my diet. A great motivation tool
Great tool
Very useful tool if you are a patient with kidney disease and may need help figuring out whether dialysis or transplant is best for you, or for a doctor or nurse to use with patients to help them understand options.
Nice tool
Simple to understand and use; presents the analysis in a clear pictographic format. Does what it sets out to do--nicely done!
