Description

This app is for high-school aged students with disabilities with an IEP, ages 14 and older. An IEP (Individualized Education Program) details your special education services. After completing a brief 10 minute survey, the app produces an individualized “Draft Transition Plan,” based on your answers. A “Transition Plan” is an important part of your IEP document. A “Transition Plan” is your road map to transition to adult life after high school. You can then email a copy of your draft transition plan to yourself or others.

A transition plan helps you move into adult life after high school. It helps you reach your dreams. Your school must develop a written transition plan as part of your Individualized Education Program (IEP). Having an effective transition plan is important. It helps you get the right services and supports so you can do what you want after high school. A transition plan can help you do things like get a job, train for a job, continue your education, and live independently.

The app also has a Frequently Asked Questions (FAQ) section that provides more information about transition services you may be entitled to, IEPs and other information about special education and related services.

This app is intended to serve as a guide to Kansas students, parents and educators on transition planning based upon requirements of the Individuals with Disabilities Education Act and the Kansas Act for Exceptional Children. Individuals residing outside of Kansas may use this free app, but it is recommend that you first check your state laws to ensure the information is applicable in your state. It’s always a good idea to consult with your state’s Protection and Advocacy System (P&A) to find out how the tools of this app may be applicable in your state (go to www.NDRN.org to find the P&A in your state). Useful, straightforward tool

While this is meant for students in Kansas, this will provide a useful framework for transition that can be emailed, printed out and shared with any IEP team. It asks the student about things like special interests, and strengths, career aspirations, future living arrangements and more to help ensure those needs and desires are planned for accordingly.
