Mother Goose Health helps patient, providers and payers initiate care and identify pregnancy-related risks earlier, then develops and executes data proven and provider led personalized care pathways reducing risk and improving pregnancy outcomes while closing care and health equity gaps.
The Mother Goose proprietary platform provides a personal approach to health equity and connectivity between the pregnant individual, healthcare provider, health advocate, and health benefits provider. This is done using high human touch closed loop care coordination services on a tech enabled AI platform. The Mother Goose Health platform is designed to improve pregnancy outcomes and reduce clinical risk by paying close attention to factors, including the wide range of clinical issues and social determinants of health, that lead to adverse pregnancy outcomes.
