This app will help you organize, manage and share your medication list.
MyMedlist stores and keeps track of your medications. The app allows the user to enter medication, doctor and pharmacy information. The data is stored on the user’s device storage, using unencrypted, persistent, key-value storage system. The app is available offline and does not require any login.
PS: Share by SMS is not functional in this release.

Pull out the app to show the list to doctors or pharmacists.
Needs some updates like desktop version for seniors and also printable format to generate a list , which patients can carry to their doctor .
