CHAMPS is designed to electronically track and trend client evaluations/quality measures, Home Health Aide evaluations/satisfaction, and quality of service delivery. CHAMPS was developed by NYFSCHAS to document and support quality and value-based services received through a series of electronic surveys designed for clients and Home Health Aides. Those surveys are described below:

• Client Evaluation Survey: This survey is designed to measure a client’s experience with their HHA and with services provided. The information may be used by NYFSCHAS to document and support the continuation of services provided and demonstrate the receipt of quality and value-based services. This survey shall be administered by a nurse and answered by the client. If the client is unable to provide a response, the survey may be answered by a family member, representative, guardian, or another person as designated by the client.

• HHA Evaluation Survey: This survey is intended to measure the HHA’s experience as an employee of NYFSCHAS. This survey is also intended to provide HHAs with the opportunity to share thoughts and ideas that may further enhance their overall work experience.
