Can’t select .md files
I was a bit surprised that I couldn't select files with the suffix ".md" in the file selection dialog.

Hello! Thank you for the review. This is a fantastic call out. We have some incredible updates happening in a few days that will add a lot of features to the app.
To address your question - Smackdown does not override defaults for Markdown files, as we strive to maintain our users preferences for how they wish to handle these types of files, but can be made to do so quite easily.
To make Smackdown the default: please right-click a .md file → Get Info → Change "Open with" → Select Smackdown → Click "Change All...".
