The Smart Care 360 App will support your participation in our preventative health and wellness programs. The App can help you stay aware of your activity, sleep, weight and other information specific to the wearables you are using with your program. You may also share this kind of information from Apple's Health app, including the steps from your iPhone. Self-reporting can also be done on the Smart Care 360 App if desired. You may also be able to communicate with your health coaches and care team, set up reminders, participate in simple wellness challenges, privately connect with others, and more. This App will not provide any diagnoses or treatment. Please contact your program coordinators if you need help with registration or setting up Smart Care 360.
