銀髮族返家照護管理APP，串連住院至返家的銀髮族照護流程，持續監測身體健康狀況，協助個人健康管理。
智慧銀髮族照護平台集結四大服務項目：
1. 出院個管照顧服務，包括：基本照護與用藥等衛教資訊、門診預約
