Added busyness to the hospital scene. Improved NPC avoidance of player. Reduced likelihood the NPCs will walk through the nurse's station area. Score card items now have a "best" flag - which indicates to the player if that option is part of the optimal playthrough. Scenario authors can tag the optimal playthrough.

Added a buy button to the main menu, to more easily access subscription options/unlock scenarios. Fixed issue where player could face the wrong telephone in the hospital scene. Made it clearer in the scorecard who the player had administered medications to. Fixed the fridge temperature label so it doesn't show through walls.
