1. Care team members can now access the Care Team Chat from the patient queue, for individual patients. 2. Any comments made by the nurse in the intermediate screen before escalation, is now logged in the care team chat as a message from the nurse. 3. When integration is enabled for any unit, we have introduced a 'Sync Now' option, to perform a manual sync with the EMR at any point in time.

Entering height is now introduced as a step in CKD onboarding. In the Event to response configuration, for each of the events, the Role of the Care Team Member can be assigned. A new question is Included in the Check-in screen to track medication adherence for a patient on a daily basis. Now, Patient Tracker & User Feedback data are available in RAD. Now the Nurse can add their availability in the Profile section for the Hour-to-Hour configuration. The Care Team members can choose from a list of pre-defined responses to send to the patients.

The following are changes reflected in the signup flow (Trial flow) for the Care Team Application across all programs: Patient list for the Doctor's role is available. Care Plan PDF is available for Jane Smith. Primary Doctor is reflected for Jane Smith. Patient Tracker & Program trackers are operational. Medications for Jane Smith in CKD Application are available.

HIS (Health Information system) Integration enabled. The Care team can now fetch patient data (Patient Demographics, Patient Conditions, Patient Medications, and Upcoming Appointments) while onboarding the patient with their unique MRN. Patient Cohort Management : Clinicians can manage multiple patient cohorts within the app. Program managers and Admins can collaborate with colleagues to ensure the patient receives the optimal care and control overall success metrics of the program. Patient Health Tracker : The app can display information about individual patients and visualize data trends. A tracker that shows all relevant monitoring data points in one place. Clinicians can catch abnormal data trends or analyze root causes by doing cross data analysis. Request and Task tracking: The app has better request and task tracking and management functionality.
