FORMIS Concussion Lite provides offline, secure concussion assessments and PDF reports for medical professionals, without collecting personal data. FORMIS Concussion Lite is a professional reference and documentation tool designed to support qualified medical professionals in the assessment and documentation of concussion and head injury. The app provides offline access to internationally recognised concussion assessment tools developed by the Concussion in Sport Group (CISG), allowing clinicians to complete assessments electronically and generate locally stored PDF records for professional use.
