DisabilityLog is a private daily journal for people applying for or maintaining SSDI, SSI, Long-Term Disability, or VA benefits. Record daily abilities, good and bad days, appointments, medications and personal notes. Create a PDF to review with your attorney or representative.
Seven daily reference categories organize selected topics from SSA Form SSA-3373: personal care, meals, housework, going out, social, concentration, and stress. They are not a complete copy of the form, and App ratings are not official SSA scores.
PRIVATE LOCAL RECORDS

- PDF generation runs on-device with PDFKit. Reports never leave your phone unless you choose to share them.

DisabilityLog is a personal documentation tool. It does not provide legal or medical advice, determine disability eligibility, communicate with SSA or any insurance carrier, or auto-fill any government form. The app does not file forms on your behalf. Ratings are personal assessments; saved categories may include unedited defaults.
PRO OPTIONS
- Annual subscription, with an introductory trial only when Apple confirms eligibility and an applicable offer.
- Lifetime: one-time purchase, when available from Apple.
Existing monthly purchases remain supported. Current localized prices and offers appear in the app.
Subscriptions auto-renew until canceled. Renewals occur within 24 hours before the period ends unless canceled at least 24 hours prior. Payment is charged to your Apple ID. Manage or cancel in Settings > Apple ID > Subscriptions. Apple handles refund requests under its policies and applicable law.
