StarlingHF is a fast, offline reference for the hemodynamics of advanced heart failure and mechanical circulatory support. Built by clinicians for clinicians and trainees, it turns the right-heart catheterization numbers you already have into the derived indices and risk scores used in advanced HF, MCS, and transplant evaluation, each shown with its formula, interpretation, and primary reference.
StarlingHF is an educational reference tool, not a medical device. It does not diagnose, treat, or make recommendations for any individual patient.
WHAT'S INSIDE
Calculators: Myocardial Performance Score (MPS), cardiac power output and index, aortic pulsatility index, cardiac index, systemic vascular resistance, MELD-XI, and body surface area. For the right heart: pulmonary vascular resistance, PAPi, RV Myocardial Performance Score, RAP to PCWP ratio, RV stroke work index, and the EUROMACS-RHF and Michigan RV failure risk scores. Dynamic testing: the milrinone challenge for LV coupling and myocardial reserve, and vasodilator reversibility for pulmonary hypertension.
Pathway: a step-by-step walk through published advanced heart failure referral logic, from the I-NEED-HELP markers through acuity, INTERMACS profile, and transplant candidacy screening to the advanced-therapy conversation, with the reasoning path and sources shown at the end.
Transplant: predicted heart mass donor size matching, the RADIAL score, the ISHLT 2023 donor selection criteria (with the long-taught decline rules the guideline corrects), and the ISHLT 2026 early graft dysfunction grading.
