Need a model release form for a photo shoot? Then print it, share it or save it to Files. • FOUR RELEASE TYPES — adult model, talent / performer (name, likeness, voice and performance), minor model with parent or guardian, and property release. • YOUR DETAILS FILLED IN — photographer or company, subject name and address, plus optional date, location, project name and consideration (for example “$1” or “a copy of the photos”). • CLEAR LAYOUT — a titled document with Parties, Agreement, optional Additional Notes and a Signatures section with signature, printed name and date lines for each party, plus an optional witness line. • PRINT, SHARE OR SAVE — print with AirPrint, share the PDF, or save it to Files. • NO BLANK GAPS — anything you leave out is simply omitted from the wording, and if no consideration is entered the standard “good and valuable consideration” phrase is used.
