ABC logs for caregivers

Free · In-App Purchases · Designed for iPad. Not verified for macOS.

Track difficult moments, spot behavior patterns, and create clearer notes or PDF reports to share with school, carers, therapists, or support teams. Behavior Tracker helps parents and carers record difficult moments, spot patterns, and create clearer notes to share with others. Quickly log what happened before, during and after an incident using an ABC-style format: - Antecedent: what was happening just before? - Behavior: what did you see? - Consequence: what happened next? - Severity and duration - Location - Early signs - What helped - Notes Behavior Tracker is designed for families supporting children who may have meltdowns, self-injurious behavior, distress, transitions, denied access difficulties, sensory triggers, or other challenging moments. Reports & Insights Unlock Reports & Insights to view weekly and longer-term patterns, including: - Entries over time - Average severity - Average duration - Common antecedents, behaviors and responses - Situations that often lead to incidents - Trends that may show whether things are improving You can also generate shareable PDF reports from your logged incidents. Reports can include incident summaries, charts, antecedent-behavior summaries, common situation summaries, average severity, average duration, and recorded patterns over time. Reports may be useful when preparing for conversations with family members, schools, carers, therapists, consultants, or behavior specialists. They are based only on the incidents you record and do not provide clinical conclusions or advice. Common situations You can add common situations, such as morning dressing, waiting for dinner, bedtime, or leaving the house, and estimate how often they usually happen. This helps compare incidents with routine frequency, giving a clearer picture than incident counts alone. For example, 3 incidents in a week may mean something very different if the routine happened 3 times or 30 times. Export and share Export logs or create PDF reports to help prepare for conversations with family members, schools, carers, therapists or behavior specialists. Behavior Tracker is not a medical device and does not provide diagnosis or treatment advice. It is a practical logging and reflection tool to help families record what happened and notice patterns over time.
