It connects you to a care team dedicated to improving your health. It is intended for preventive care, or follow-up of a chronic pathology or after a hospitalization. As a patient, you will be able to periodically report information requested by your doctor. Vital signs, health metrics, questionnaires, symptoms, all parameters that will help your care team to individualize your medical follow-up. You also benefit from all the tools intended to take your health and well-being in hand in a single place: appointment booking, video-conference consultation, taking of vital signs by the camera, integration of connected objects, emergency button or even a secure messaging system to communicate with your team.
