Hospital Passport App
When using Hospital Passport, you (or your carer) can log information about yourself and your conditions.
You can generate a PIN and share that PIN with a clinician, along with your name and Date of Birth. This will authorize the clinician to view all of the information you have entered about yourself in the app.
