This app is designed for a patient to fill out paperwork in an outpatient physical therapy setting without the need of paper. That information can then be digitally transferred to the clinic's computer without the need of printing paper, having the patient fill it out, and then scanning it back into the computer. This can help reduce the cost of paper and ink as well as streamline the process of collecting paperwork. It has both Spanish and English capabilities. Disclaimer: You are ultimately responsible for your own HIPAA compliance. This app follows the necessary encryption requirements for storage of health information but keeping passwords private and protected is necessary as an user. You are also responsible for transferring of any information. You may use whichever transfer method you deem appropriate but the safest way is directly to your computer via a lightning cable.
