When the app starts, the display brightness is set to maximum. When you exit, it is adjusted back to the original brightness. The app has a white background, so it serves as a light and you can already see in the preview if it is bright enough. No additional LED lighting is required. When you press the shutter button (button or volume buttons), a timer of 5 seconds runs until the photo is taken. You can use the iPhone in portrait or landscape mode, the photo is always correctly oriented.
