Clinic@Home breaks down barriers to access for essential health and wellness services so that Minnesota Medicaid waiver recipients can live independently in their community and access care on their terms. Existing Clinic@Home clients can use this application to track medication adherence, view health trends, review upcoming Clinic@Home appointments, and access their custom education library.
This version introduces device reminder notifications to help you use your Clinic@Home monitoring devices at scheduled times for improving compliance. You can also configure device reminder notifications in Settings page.
