If you’re a patient at any of our hospitals or doctors’ offices on Long Island, you can use the Catholic Health app to manage your health from anywhere.

Log into the app with your Catholic Health MyChart username and password to:

-Find a doctor.
-Schedule and manage appointments.
-Check in to your appointment virtually.
-Request a prescription refill.
-Communicate with your care team.
-Review your test results, notes from your care team, medications, immunization history, and other health information.
-View and pay your medical bills.
-Upload health and fitness data, including data from the Apple Health app, when enrolled in self-tracking programs.

If you don’t yet have a Catholic Health MyChart account, visit mychart.chsli.org to sign up for an account, or call 631-465-6100 to get an activation code.

Care

I was in good Samaritan yesterday, August 12 in the emergency room came with ambulance. I had dizziness lightheadedness I had passed out and my husband got me. I didn’t hit anything they did all test, CAT scan bloodwork etc. but I never got an answer of what they at least thought it was, it’s hard to understand all that they say to you in my app can you please have someone explain this to me maybe you can send me an email it is on file

Dr lied Why would he do that? I asked for the cocktail

I swear on the lives on my children I didn’t ask for narcotics repeatedly. I asked for it initially because the Percocet wasn’t working and I stopped taking my Buponorphine for a week. I would never ever ever ever swear on my children’s lives unless I was telling the truth. I asked for the Tylenol Magnesium Benadryl and Reglan But he was adamant about not giving it to me . I also told him that Dr Strumpfler gave me IV Compazine once and he wrote for Dilaudid if the Compazine didn’t work well look in my chart it worked and I didn’t need the dilaudid. I never took the Dilaudid . Look in the chart. Why would he lie? Why would he lie? I don’t understand Why would he lie?

Instantly updates as the office does it

This app sent me a notification just as the office updated my information. That’s the first time I’ve have an app provide me real time updates regarding my appointments and information. Very simple to view all appointments, doctors and information on what I need to still schedule.
