Fixed an issue with starting an appointment when one was already started.

* Several minor bugs were fixed * When start and completing an appointment, modal pop-up window now appears to let the user know that something is happening (as it takes several seconds to make the round trip to the server). This will prevent the user from accidentally clicking on the buttons to start and complete an appointment more than once. * When sending messages via app, the text field for the message will now stay above the onscreen keyboard while type and will be at the bottom of the screen when not actively typing.

New design for log in page and messaging page. Removed reset and pause functionality. Fixed bug where disabled icons were not disabled on appointments for iOS.

Removed support for email login. Added support for phone number login.

Fixed an issue with multiple appointments being active at same time. Location tracking features to happen less often. Removed dependency on third party location service.
