Bug fixes to memory related issues

-Fixed validation error for shoe only order -Activity and Weight fields disabled for Shoes Only / No. 1 / No. 2 -Photos disabled for Shoes Only / No. 1 / No. 2 -Added patient name to the Insert Type change notification -Orders now are forced to have a patient name to be saved.
