Added support for new custom appointment types

In this release we improved the logic for Wellness plan subscriptions, by adding functionality allowing users to upgrade or add to existing subscriptions. The front-end was enhanced accordingly and now can filter and exclude certain plans based on user's existing subscriptions and active plan versions.
