Delirium Wheel

This App has been developed to support care professionals to help them detect delirium and also help them manage people suffering from Delirium by enhancing their awareness, skills and knowledge in this area. This is an app that aims to help and enable care home and community staff to swiftly identify residents who are suddenly deteriorating with an underlying delirium. Staff will be able to use the app to detect various types of delirium and also utilise the Delirium Wheel to identify several underlying causes and create a non-pharmacological management plan to support patients suffering from delirium. Staff can access various resources through this app, including NEWS2 and 4AT, which is a validated delirium detection tool. This app has been commissioned by the NHSE/I transformation arm to support frailty collaborative initiatives across the West Midlands. System representatives identified the need to develop a responsive delirium pathway to support care homes in reducing unwarranted attendances to Emergency Departments (ED)
