It would be helpful if this app had a few more features. There is no export, so no creating reports or sharing entries to other applications or with other people (such as your team). For things marked “try” there are no options to indicate which you may have tried. The filters don’t have an option to filter by type (i.e., things to start, stop, or try).

Action Plans now toggle from lists or the editor, with a refreshed History view.
