Rehab Home Checklist helps document home-environment visits on iPhone and iPad. Organize photos, metric measurements, findings, guide answers and floor plans by patient and visit, then share an A4 PDF. Available in Japanese and English.
RECORD DURING A VISIT
• Select your current location and record the measurements, photos and findings you need.
• Record approach steps, thresholds, the raised entrance floor, stairs and landings individually.
• Use the optional question guide to review daily activities and assistance. Keep unconfirmed and deferred items visible.
• Entries save automatically. Resume an unfinished visit where you left off.
• Previous values show their date and source separately from this visit’s measurements.
FLOOR PLANS AND REPORTS
• Create a manual plan with rooms, corridors and doorways.
• On supported devices, scan rooms or a home, review separate levels and 3D, and save the result.
• Preview and share handoff or complete PDF reports with photos and plans.
• Names, addresses, room names, findings and notes you enter are never automatically translated. Changing language preserves stored values, metric units and level numbers.
• Open Help → Show sample to try a fictional visit.
DEVICE REQUIREMENTS
Requires iOS or iPadOS 18 or later. AR measurement requires an ARKit-compatible device and camera permission. Floor-plan scanning requires a LiDAR-equipped iPhone Pro or iPad Pro supported by RoomPlan. Manual entry, photo import and manual floor plans remain available without scanning or camera permission.
ONE PURCHASE, ALL FEATURES
A one-time paid download with no in-app purchases, subscriptions or ads. Your local App Store displays the price. Existing users do not need a new feature-unlock purchase.
DATA AND USE
Records stay on your device. No account is required. The developer does not collect or automatically upload patient records, and there is no advertising tracking. PDF reports are passed to the destination you choose when sharing. Depending on device settings, stored records may be included in device backups.
AR measurements and scanned plans are estimates. Verify dimensions needed for alterations or safety with a tape measure. This is a documentation tool, not a diagnostic or safety-assessment system. Some checklist items refer to Japanese housing and systems; the app does not certify compliance with building or safety standards in any country.
