AdultReady: Transition Plan
Adult goals into real progress
Free · In‑App Purchases · Designed for iPad. Not verified for macOS.
Adult life is not one goal. It is a connected plan for learning, work, daily living, transportation, community participation, and the support that makes progress possible.
AdultReady helps young adults and the adults supporting them turn broad hopes into practical, measurable next steps. Start with strengths, interests, preferences, communication needs, and a self-advocacy statement in the young adult's own words. Then connect each goal to a baseline, target, responsible person, date, progress evidence, and next action.
Use AdultReady to:
- Build a strengths and preferences profile that leads with the young adult's voice.
- Organize goals across education, employment, independent living, transportation, community, and support services.
- Record skill practice with context, support level, confidence, evidence, and the next step.
- Track programs, applications, requested documents, contacts, deadlines, and facts that still need verification.
- Prepare meeting questions and record decisions with a clear owner and due date.
- Create a transition meeting packet with goals, progress, programs, responsibilities, and open questions.
AdultReady keeps records locally on the device. There is no account, analytics, advertising, or tracking. Support is available through a private in-app contact form.
AdultReady is a planning and record-keeping tool. It does not provide medical, legal, benefits, education, or eligibility advice. Verify requirements and services with the relevant organization.
