This application estimates probability of survival in patients with chronic heart failure. Using the application to calculate risk involves entering four simple characteristics of patients with heart failure, including medication use, symptom status, exercise capacity, and a widely-used blood test in heart failure. Survival estimates are based on data analysis of 2255 patients with chronic heart failure managed at a specialized heart failure clinic, the Ahmanson-UCLA Cardiomyopathy Center. This application is for use by physicians who care for patients with chronic heart failure.
