HUBCHART app is a free HIPAA compliant, Patient-Centered Collaborative Care app. It is the best tool to manage thousands of Patient Care Groups. The Care Team can securely share vital information with each other anytime, anywhere. This Team Care approach helps facilitate and expedite care delivery. The “Patient-named” Collaborative Chat Groups can function like an ad-hoc Health Record where all chats, files, images, videos, and link can be shared within the secured group and archived for future reference. These private group chats are managed by a group owner and administrators, as well as other participants of the care team permitted by the group owner. The Care Team can be made up of doctors, doctor’s staff, the patient or family, Pharmacy etc. Hubchart’s Patient Centric Groups are especially valuable for Complex Medical Care, Case Management and Concierge Medicine where individualized care can be delivered by a multidisciplinary team continuously coordinating with each other to provide optimum care. Aside from better healthcare, Care coordination can be billed by providers using Chronic Care Management billing Codes and other Non Face to Face billing codes.
