a. Add option - I do not have my Member ID
i. If "I do not have my Member ID" is selected, then Member will be asked to provide their name, DOB, phone number
ii. Send the provided information via email to, Andrea Travis, and Mike Bechtol
iii. Provide a confirmation message "We have received your request and will reach out within 24 hours
2. Member Information Updates: If Firstname, Lastname OR DOB is update, Provide confirmation message "Request to Update Your Member Information Received Successfully". Track the changes so that information is not missed by RDH staff.
3. Added options on home screen to call Tollfree number or send TEXT message to get help
